Notice of Loss
  1. Download the Notice of Loss form and fill it out.  
        
  2. Fax a copy to (863) 644-3478 and mail the signed original copy of the Notice of Loss form to our office at:

    Lakeland, FL  33807     
        
  3. Call our office at (863) 647-2747 to notify us that a loss has been filed.

Crop insurance is what we do. It's all we do.
